New Patient Form
We ask that you arrive 10 minutes prior to your scheduled appointment to complete our New Patient form.
Initial Examination
During your first appointment, your dentist will review your medical and dental history. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening, and any necessary X-rays.
Treatment Plan
Based on your history and findings from your examination, your dentist will develop a long-term dental treatment plan for you. They will also answer any questions you have about your dental health and discuss treatment options.
